We are so happy you have chosen Alabama Pediatric Therapy Services, LLC. for your child’s therapy services! The mission of APTS is to provide the highest quality therapy services in an environment that is compassionate, respectful, and supportive to our clients and their families.

Before your child’s first visit, please have the following information prior to scheduling your first appointment:

• Insurance information so that we can verify your level of benefits such as deductibles, co-pays, co-insurance, maximum out of pocket expenses, whether a prior authorization is needed for services, and if there is a limit on the number of visits your plan allows. You will know, if any, your expected out of pocket costs prior to your first appointment.
• The referring doctor’s name so that we can contact them to obtain a prescription for the evaluation/therapy services needed unless one has already been provided to you. If you have the prescription, we will need a copy of it before we can schedule your first appointment.
• If your child has had therapy services previously, or currently has an IEP at school, please bring these reports to your child’s evaluation. We will make a copy for your child’s file. This information will help ensure the evaluating therapist has accurate history of your child’s progress, and assist in developing a clear picture of your child’s current level of function.

How to prepare for your first visit:

• Download the patient intake form for the office location you will be visiting, complete it fully and bring it with you.
• Watch the social story video for the office location you will be visiting to prepare your child for their visit.
